Our practice participates in most major dental and many medical health insurance plans. If you are covered by one or more dental /medical insurance plans, please bring any forms to the office at the time of your first visit. As a courtesy to our patients, we will submit dental and medical insurance forms on your behalf.
Responsibility for payment of deductible, co-payments and outstanding balances remains your obligation.
If you have any questions or concerns regarding insurance or fees and payments, please do not hesitate to discuss them with our front desk staff prior to your treatment. We will be happy to provide you in advance with an estimate of proposed fees for any of our procedures.
It is the policy of this practice to ask the patient to pay any deductible, co-insurance or any amount not covered by the insurance companie(s) at the time service is rendered. Payment can be made by cash, check, Master Card, Visa, Discover Card and CareCredit. Please note, starting January 2024, our offices will be adding a 3% credit card transaction fee charge. This will not affect those who wish to pay by cash, check or care credit. Our team will continue to monitor the situation and we look forward to any constructive suggestions and feedback.
We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. If you have questions regarding your account, please contact us . Many times, a simple telephone call can clear up any misunderstandings.
Please note, beginning March 31, 2024, we will no longer be participating in Medicare. We understand the inconvenience that this may create and the leadership of the practice will continue to monitor the situation. We appreciate your understanding.
